Scoring significant findings — frameworks and pitfalls

Getting rating consistency between assessors is notoriously hard. Some firms use a 1-5 severity scale, some use RAG, some use a matrix of likelihood × consequence. What works for you, and what have you learned about communicating ratings to responsible persons who are not fire-safety trained? Clients often struggle to translate "significant" into "urgent" when the action language does not match their risk vocabulary.
